Self Sufficiency Program
 
Who is eligible to receive assistance?
  • Must be a resident of Bath, Menifee, Montgomery, Morgan or Rowan County. Client must apply in the county they reside.
  • A minimum of two (2) case loads will be active in each county. 
  • Household income must be 200% or below the Federal Poverty Level.
 
What type of information must clients provide?
 
  • Recent copies of utility bill if for utility assistance or if for shelter/rent assistance a statement from landlord
  • Proof of income from previous month for all members of the household. Acceptable forms would include: payroll stubs, Social Security Award letter, unemployment benefits, retirement, pension, disability, etc.
  • Proof of physical address; can use driver’s license, utility bill, or statement from 911 coordinator verifying address.
 
 
How much assistance will be provided?
 
Self Sufficiency Program Outreach Worker provides encouragement, guidance and referrals to clients who are interested in making positive changes in their lives. 
 
Benefits includes the knowledge of the Outreach Worker of community resources and referrals, financial support, training and just having someone that is on “your side” and will advocate on the client’s behalf. Financial support per person can be obtained up to $300 per 2009-2010 program year as deemed necessary upon the assessment scale of needs and at the discretion of the outreach worker based upon what is needed in eliminating barriers that keeps the client from being self sufficient. 
 
The Self Sufficiency Program is a joint effort between the Self Sufficiency participant and Outreach Worker to set goals and milestones that will improve the participant’s self-sufficiency and present income level.

This Self Sufficiency Program will operate on a 1, 2, 3 STEP process according to the needs of each participant on an individual basis. Clients may go back and forth from steps depending on current severity of needed support by the Outreach Worker.    
 
STEP 1:  During initial face to face meeting, the Outreach Caseworker will conduct the intake process of: Assisting the participant in setting goals and milestones that will improve self-sufficiency by completing a Scale Assessment, Progress Achievement Plan, Program Agreement form, Release of Information form, client rights policy and establish goals toward improved income management, collecting the proof/documentation of income, proof of identification and other required information. The Outreach Caseworker will then schedule a follow up face to face within the next month.  
  
*Have face to face contact with the participant one (1) time per month at which time all goals will be reviewed and adjusted as needed, and one (1) phone contact per week to discuss progress towards goals.     
 
*Support participant while working toward goals by offering support services and linking to other possible community resources (referrals).
 
STEP 2: If after a few months of involvement with the participant and they are working well independently on their goals, you may consider entering into Step 2. This step will allow the monthly contact time to keep your client motivated, encourage continued goal work, and address obstacles.
 
Have face to face contact with the participant (1) time per month at which time all goals will be reviewed and adjusted as needed, and 1-2 phone or follow up by letter contacts per month. 
 
► STEP 3: This step should be entered into only if the participant is working on long term goals at a steady pace independently with minimum support by the Outreach Worker. This will be towards the end of the participant/Outreach Worker involvement. However, you want to be careful not to enter this step if the client still needs case management to achieve their goals. The completion of the Self Sufficiency Program will be determined by whether or not the client has achieved all set goals and has agreed or requested to be exited from the program. 
 
*One (1) phone follow-up per month.
